Life-Threatening Healthcare Plans

Dear parents/guardians,

If your student has a health plan for a life-threatening health condition such as anaphylaxis, seizures or diabetes:

Your school nurse will send care plans home in June so that you can have them completed over the summer and signed by a doctor.   Plans and medications are due to the nurse two weeks before the first day of school to allow time to process and have the information ready for the first day of school.  Any healthcare plans and medication received after the deadline may not have their child begin school on the first day.  Please plan accordingly so that this very important work can be done.
