Dear parents and guardians,
Each academic year we require new paperwork for medication orders and healthcare plans.
If your student has a life-threatening condition, be sure to make an appointment with their healthcare provider this summer so that everything is in place by the first day of school. Please bring medication orders and life-saving medications with a completed healthcare plan to the IMS office by August 14.
For lifesaving conditions such as anaphylaxis, diabetes and seizures, your student will not be allowed to start school until paperwork is completed and medication is in the building.
If you currently have medication in the health room, you may pick it up on the last day of school as your student leaves the building.
